WHAT IS ANORECTAL MANOMETRY?
Anorectal Manometry (sometimes referred to as anal manometry or rectal manometry) is a set of pain-free tests that allow us to evaluate the pelvic floor function of men, women, trans, and non-binary individuals. The test is administered by positioning a thin, flexible catheter into the anus that feeds through to the rectum.
The variables most commonly measured are the anal sphincter resting pressure, the anal squeeze pressure as a mark of muscle strength, the Rectoanal Inhibitory Reflex, rectal sensation tone and compliance, simulated defecation, sensation, discrimination, fatigue, and paradoxical contraction.
WHY IS ANORECTAL MANOMETRY NEEDED?
Anorectal Manometry is necessary for some patients to evaluate pelvic floor function and uncover potential issues that may be causing unwanted symptoms. Symptoms that may require Anorectal Manometry testing include anal fissures, anal tightness, anal looseness, constipation, fecal incontinence, difficulty defecating, non-complete emptying, pain in the pelvis, pre- or post-surgical interventions, pain with or the inability to participate in receptive anal sex, and having a weak unsatisfactory orgasm.

If you have had a normal bowel movement before taking the test, you will be fine. There is no need for an enema, nor is there a need to alter your diet. Balloon Expulsion Tests
During this test, a thin, lubricated catheter with a small balloon is comfortably inserted into the rectum while you lie on your side. You’ll be asked to perform a series of simple tasks, such as:
- Resting quietly to measure baseline anal pressure
- Squeezing and lifting as if stopping gas to assess external sphincter strength and endurance
- Attempting to expel the catheter, both with and without the balloon inflated, to measure defecation biomechanics
- Responding to gradual balloon inflation, which tests rectal sensation, reservoir function, and reflexes that help maintain continence
This test also evaluates the Rectoanal Inhibitory Reflex (critical for bowel function) and the Cough Reflex (which protects against leakage during sudden pressure increases). Afterward, the catheter is removed, and you can return to your daily routine immediately.
Anal Sphincter Electromyography (EMG)
Electromyography (EMG) uses a small lubricated anal probe to record the electrical activity of pelvic floor muscles. While viewing results on a computer screen, you’ll be guided through maneuvers such as resting, squeezing, lifting, pushing, and coughing. These actions are displayed as line graphs, showing how your muscles contract, relax, and coordinate.
EMG is both diagnostic and therapeutic. It identifies muscle weakness, asymmetry, or hypertonicity, and it can also be used for biofeedback therapy. In biofeedback sessions, patients gain real-time awareness of their pelvic floor activity, helping to:
